SmartAlign.net is a consulting service designed to help you achieve a predictable case
finish for your patient’s Invisalign treatment and to help establish a result that will
provide your patient a lifetime of Fit, Function, Stability & Aesthetics. Your professional
responsibility is to perform a complete examination, diagnosis, treatment plan and
coordinated dental care to properly finish and maintain.
SmartAlign.net has several options to personalize your patient care:
Case Selection & Review - Fee: $75
This service is designed to review your patient records prior to submission, to help
you determine if Invisalign can in fact be a good course of treatment for your patient
and which treatment option would suit the case. This option can also be used if you
would like an evaluation of a case in progress to determine the need for a refinement.
You will need to email the photo series and panorex to DrRob@smartalign.net once
you have finished this form submission.
Comprehensive ClinCheck Support - Fee: $275
This service is designed to provide you with a completed ClinCheck with the Invisalign technicians. Your patient records and initial prescription must be submitted to Invisalign through your site. I will then access your site and provide all ClinCheck prescription instructions and changes until the ClinCheck is designed with an optimal and predictable finish. You will be notified by email when the ClinCheck is ready for your final approval.
Refinement or Mid-course Corrections (A) - Fee: $125
This is for a case initially started with SmartaAlign.net consulting. Please email your initial photos and the most current photos for my evaluation to DrRob@SmartAlign.net.
Refinement or Mid-Course Corrections (B) - Fee: $275
This option is for a case not intially started with SmartAlign.net. If your patient’s case did not finish with the results you were expecting and you would like SmartAlign.net to do the refinement, this is the option for you. Please email your initial photos and the most current photos for my evaluation to DrRob@SmartAlign.net. You must submit new impressions and photos to Invisalign before I can provide the refinement ClinCheck prescription instructions and changes to provide you with a refinement that will provide your patient with an optimal and predictable finish. You will be notified by email when the ClinCheck is ready for your final approval.
Consultation Services
Click on the “Begin” button below to be direct to a form to fill-out. Please fill this out first. Once submitted, you will be given your application fee options. All transactions will go through PayPal. Your order is not complete until you complete the selection and checkout process through PayPal. Dr. Leach will then evaluate your case and the consultation process will begin. Prior to submission, be sure to have your patient sign the HIPPA compliance forms before submitting any information to SmartAlign.net.
Doctors using this service must agree to our Consultation Agreement.
